How Semaglutide and Tirzepatide Work
Semaglutide and tirzepatide are once-weekly injectable medications that mimic natural gut hormones involved in appetite and blood sugar control. They help you feel full sooner, reduce cravings and “food noise,” slow stomach emptying, and improve how your body handles insulin.
Semaglutide is a GLP-1 receptor agonist.
Tirzepatide is a dual GIP and GLP-1 receptor agonist, meaning it acts on two appetite- and metabolism-related hormone pathways.
The semaglutide and tirzepatide we prescribe are compounded medications, prepared by a licensed compounding pharmacy for each individual patient. Compounded medications are not FDA-approved.
Why Medical Supervision Matters
GLP-1 medications work best, and most safely, with the right dose escalation, nutrition support, and monitoring. Rapid weight loss without enough protein and strength training can cost you muscle along with fat. That’s why every New Theory Medical patient gets regular body composition analysis, not just a scale weight.
Who Is a Good Candidate for GLP-1 Weight Loss?
You may be a candidate if you are an adult with:
- A BMI of 30 or higher, or
- A BMI of 27 or higher plus a weight-related condition such as high blood pressure, prediabetes, high cholesterol, or sleep apnea
Not sure where you fall? Check your BMI or simply book a consultation and we’ll evaluate you in person.
These Medications May Not Be Right for You If You:
- Have a personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2)
- Are pregnant, planning pregnancy, or breastfeeding
- Have a history of pancreatitis
- Have certain gastrointestinal, kidney, or eye conditions
A full medical history review is required before any prescription.

What Results Can You Expect?
First Few Weeks
Many patients notice reduced appetite and fewer cravings as treatment begins.
First Few Months
Weight loss is gradual and builds as your dose is adjusted, with body composition checks to make sure you’re losing fat, not muscle.
Long Term
A maintenance plan helps you keep your progress, since weight regain is common when treatment stops without one.
Results vary from person to person and depend on dose, consistency, nutrition, and activity.
Side Effects & Safety
Common Side Effects
Nausea, diarrhea, constipation, vomiting, heartburn, and fatigue are the most common, especially when starting or increasing a dose. They usually improve over time. Eating smaller meals, staying hydrated, and avoiding greasy foods help.
Less Common but Serious Risks
Pancreatitis, gallbladder problems, low blood sugar (especially with insulin or sulfonylureas), kidney problems from dehydration, and allergic reactions. Contact us or seek care right away for severe abdominal pain or signs of an allergic reaction.
Important Safety Information
Semaglutide and tirzepatide have been associated with thyroid C-cell tumors in rodent studies. It is unknown whether they cause these tumors in humans. Do not use if you or a family member has had medullary thyroid carcinoma (MTC), or if you have MEN 2.
